* Please read the waiver. It must be signed by one of the child's parents on the 1st day of participation.

This card is not refundable. However, If the current cardholder decides to stop the activities within our school, we give him/her the option to transfer the remaining membership to another person.
Member agreement and acknowledgment of risk
In consideration of the services of Uplay Martial Arts Luxembourg, I hereby agree to release and discharge them as follows:
a) I acknowledge that participation in the martial arts entails unknown and unanticipated risks which could result in physical or emotional to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. The risks include, among other things: intense, stressful and strenuous exercises with body contact.
b) I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary.
